The Policy on the Protection of Personal Information

Smart Box INC. obtains and utilizes customer’s information such as name, address, telephone number and email address in order to smoothly conduct our business. We recognize the importance of the protection of these customers’ personal information and observe the compliance program on the protection of personal information as a company which offers a total information service. We also treat the information correctly and properly and accomplish highly transparent corporate activities.

We also comply with the general and fair practice to treat personal information and properly conduct the acquisition, utilization and offer of personal information clearly setting the purpose within the necessity of our business.

■Procedures to answer the request for release of personal information etc.

The administrator of personal information (TEL:045-228-9280) deals with the request to notify and release the purpose, to correct, add or delete contents, to stop use, and to stop providing third parties. The administrator of personal information also answers complaints and inquiries on personal information. However, in the event that we are not able to disclose personal information due to stipulations of laws and ordinances or the expiration of our storage, we are not able to deal with the request to notify and release the purpose, to correct, add or delete contents, to stop use, and to stop providing third parties. If you would like disclosure etc. of personal information, please contact the administrator of personal information (TEL:045-228-9280). We will send you the application form.

The materials you need to send us include:

1.The application form to inquire about personal information: Please enter the following ① to ③ on the application form.
①The address, name, telephone number and the seal of the person in question
②he date of request
③Information to identify the request to disclose personal information

2.The certificate of the person in question or the representative: In case of the person in question:
・ Driver’s license, passport, health insurance card, or a copy of resident card
In case of the representative:
・ The proxy to identify the representative (necessary)
・ The certificate of seal impression of the person in question whose seal puts on the proxy or the document to identify the representative

*Please ask the administrator of personal information in case the representative requests it. People who may request through the representative are limited as follows;
・ A legal representative of a minor or a guardian for an adult
・ A representative who is delegated by the person in question to request the disclosure etc.
When we receive the request above, we will answer the request by written notice within two weeks following necessary examination. In answering, we use secure mail, in which only the person in question may receive in order to prevent the leak of personal information by identify fraud.
